Plans directs and conducts habilitative and rehabilitative counseling programs and discharge planning activities.
Delegates and instructs support personnel.
Employees in this position are responsible to work a minimum of two hard to staff shifts as designated by the Rehab PRN policy and have a minimum work frequency of is six shifts per the entitys posted work schedule of 8 weeks.
